Rehabilitation Team
The experienced Rehabilitation Team is responsible for assessing the individual needs of a person with ABI and for developing a person centred programme of rehabilitation and support which will help the person maximise their abilities and ultimately change their lives for the better. The Rehabilitation Team consists of clinical staff , and Local Services Managers, ABI Case Managers and Rehabilitation Assistants together with the person with ABI and family members/carers affected by ABI.

Local Services Manager
The Local Services Manager manages assisted living residences and/or coordinates Home & Community services in their area. They work as part of the Rehabilitation Team, managing a team of Rehabilitation Assistants, taking the lead in the provision of person centered services, linking in with families and the wider community.
The Local Community Services Manager supports individuals with Acquired Brain Injury (ABI) to live valued lives in their local community. They manage a team of Rehabilitation Assistants whose role is to facilitate and assist optimal independence and participation in activities within an individual’s own home or work environment and in the community.
The Local Services Manager assists people in recovering skills and confidence through a Individual Rehabilitation Plan which facilitates individuals to achieve the goals that are important to them.
ABI Case Manager
The ABI Case Manager is a professional whose role in the Rehabilitation Team is to be the one point of contact for an individual with an Acquired Brain Injury and their family. The Case Manager will seek out and coordinate appropriate resources, monitor progress and communicate with the person with ABI, the family/carer and other professionals including all statutory and non-statutory agencies.
They identify local sources of help or support, educate and support carers, explore activities that may be available, and help the person with an ABI structure his/her time and live as independently as possible within their local community.
Rehabilitation Assistant
Rehabilitation Assistants are trained partners in a programme of enablement or re-enablement.
The Rehabilitation Assistants are neither personal assistants directed by the person served, nor are they carer’s directed by the families/supervisor. They both assist and support in partnership with persons served, directing and adapting their input to the needs of the persons served.
The Rehabilitation Assistant is a key member of the Rehabilitation Team, promoting independence with the person served in their home and community. As the person served becomes more able, the Rehabilitation Assistant becomes less involved. Each person has an individualised person centered plan which is used to support the person in identifying and realising their goals. They are supervised and supported by clinicians and other qualified staff.
Rehabilitation Assistants may work with persons served in Residential Services or in Home and Community Rehabilitation Services.
