The ABI Case Manager is a professional who serves as a point of contact for an individual with an Acquired Brain Injury and their family. The Case Manager will seek out and coordinate appropriate resources, monitor progress, and communicate with the person, family, and other professionals as appropriate.
They identify local sources of help or support, educate and support carers, explore activities that may be available, and help the person with an ABI structure his/her time and live as independently as possible within their local community.
