Home & Community Rehabilitation Service
The Acquired Brain Injury Ireland Home & Community Rehabilitation Service is a follow-on service to clinical rehabilitation. It helps people with acquired brain injury to re-adjust to life in the community following a period of rehabilitation in hospital. This service assesses progress in the home environment after rehabilitation and it also incorporates an assessment process for people with ABI who have not received rehabilitation services. Community Rehabilitation Services focus on improvement of functional skills, health and medical management and teaching alternatives to behaviours that interfere with a person’s functioning and rehabilitative progress.
A personal community reintegration programme is designed for each individual with ABI. This is based on maximising ability, promoting a better quality of life and enabling the person with ABI to regain independence where possible. Each programme is designed in consultation with the clinical rehabilitation team, the person with ABI and their primary carer or family. It is carried out by trained rehabilitation assistants under the supervision of a Community Services Manager. The Rehabilitation Team is made up of Neuropsychology, Social Work and Occupational Therapy where available.
