 TRANSITIONAL LIVING

Transitional Living Service

The focus of the Transitional Living Service is to provide rehabilitation and support in a residential setting, with a view to enhancing the independent living skills of individuals who have an Acquired Brain Injury. The work is supported by a team of clinicians specializing in the area of ABI, a Local Services Manager and a team of Rehabilitation Assistants.

Each participant has their own Individual Rehabilitation Plan with clear achievable goals, which is developed in conjunction with the person served, the staff and other appropriate stakeholders.

Time spent in this service will also allow the staff team and clinicians to assess the independent living potential of the person served, and/or the level of support required either to move to their own home, or move back with their family. Length of stay is dependent on assessment of need and agreed in the Individual Rehabilitation Plan.

The nature of this service is transitional, low support, and the focus is on enabling the person to live in their own home. At any one time, four persons served will be supported by one Rehabilitation Assistant at evenings and at night. This service is provided on a 4 night/five day basis. This is to offer persons served an opportunity to implement their learning their long term environment at weekends, and use this experience as part of the learning process within the Rehabilitation process.

A Structured Rehabilitation Programme will be provided during the day. This is also available to others with an Acquired Brain Injury who live in the agreed catchment area. This programme will be based on the identified needs of participants, and each person served will have a Individual Rehabilitation Plan.

The specialist elements of the programme will be provided by the Trans-disciplinary team, and supported by Rehabilitation Assistants. Some of this programme will be delivered in the house, but Acquired Brain Injury Ireland is anxious to encourage persons served to access other places and resources in their community, and will work to secure access to other meeting places within the community
