How to make a referral
We accept referrals to our services for people living with an acquired brain injury between the ages of 18 and 65.
Learn how to make a referral for our services below
Unfortunately, we cannot accept online referrals at the moment. If you completed an online referral between November 2023 and March 2024, we ask that you please download and fill out the referral form below and return it to your nearest Area Administrator. Thank you.
Referrals must be made by a healthcare professional, for example a GP, Case Manager, hospital or community medical professional, or HSE disability service. We also accept referrals from other Community Disability Service Providers including Headway, the Irish Wheelchair Association and Enable Ireland.
If you or someone you know would like to access our services, and if you are unsure who to approach to make the referral, please phone us: +353 1 280 4164 (dial option 4).
To refer someone to our services, download our referral form at the link below. Complete the form and return it to the Area Office for your county. Area Office details are listed at the bottom of this page.
You will also find a ‘Consent for Release of Information’ form at the end of the referral document. This must be signed by the person with acquired brain injury, or by their guardian if appropriate, and returned to us as part of the referral. Please note that we cannot process referrals without an attached ‘Consent to Release of Information’ form.
Forms can be sent to the Area Office by post or scanned and emailed to the relevant address.
We welcome self-referrals by survivors of brain injury for our Step Ahead Plus programme. Referrals can be made using our online form, or by downloading and returning a paper form by post.
When you apply for the programme you will need to include a document that confirms you have an acquired brain injury. Examples are a medical report or a discharge letter.
The online referral and paper referral forms can be found on the Step Ahead Plus page of this website.
What happens when you make a referral?
All of the referrals we receive are treated with the utmost respect and confidentiality.
- Once the referral form is received, we will request information from the other health professionals referenced.
- We will then arrange an initial assessment meeting to discuss your current support needs and rehabilitation goals.
- If we cannot immediately provide the support services you need, you will be placed on a waiting list.
- In some cases, we may not be the most appropriate service for your needs. In these cases, the person making the referral will be notified of this decision.
