Case Management
Our Case Managers are highly skilled professionals, trained to guide and support brain injury survivors and their families on their rehabilitation and recovery journey.
They are an important link between the person and the services they receive, and offer knowledge, education and expertise to family members as they navigate the pathway from hospital to home.
Crucially, our Case Managers help those impacted by brain injury to avoid long and unnecessary stays in hospital, or inappropriate placement in nursing homes. Their goal is to ensure that survivors can live their lives as independently and confidently as possible.
We are funded to provide Case Management services in many counties in Ireland, but significant gaps remain especially in the east of the country. We are working hard to increase investment in this important service.
